If the 700 USD includes the product cost only, then apart from shipping cost, you’ll need to pay for #3 and #4. It’s recommended to confirmed with supplier which incoterms you used for your order. If it’s EXW, you need to cover the cost of #3 from your end; If it’s FOB terms, then your supplier pays #3,#4 is always on your side.

If you use express shipping e.g. DHL/UPS/ special airlines with door to door service, you just need to pay the air freight in one time.If the cargo value is less than 800USD, you may not be charged for duty based on this amount. (Please follow the latest US Customs policies)
